Jag och min Alzheimer

Jag har fått en Alzheimers diagnos (Kod F002/F018). Här publicerar jag texter i ett försök att bygga en faktasamling om sjukdomen. Konkurrensen är tyvärr inte stor.

Exercise as Medicine for Alzheimer's

Physical exercise is a cornerstone of the FINGER model, along with diet, cognitive training, social activity, and managing vascular risk factors. The FINGER study showed that such a comprehensive program can help preserve cognitive function in older adults at an increased risk of dementia. However, because several interventions were combined, it's impossible to say exactly how much of the effect came from the exercise alone.

That's why studies on people who already have memory problems or Alzheimer's are particularly interesting.

In the Danish ADEX study, 200 people with mild Alzheimer's exercised. While no clear, general improvement in cognition could be shown, those who truly exercised regularly and with sufficient intensity performed better on tests of mental speed and attention. The training also improved their physical fitness and reduced neuropsychiatric symptoms.

Other studies have also found connections between improved fitness and better memory results. In a study of people with early Alzheimer's, better fitness was also linked to less shrinkage of the hippocampus, the part of the brain that is central to memory and is affected early in the disease.

The large American EXERT study yielded another important result. People with mild cognitive impairment, MCI, were assigned to either aerobic exercise or lighter exercise that included stretching and balance. After a year, there was no certain difference in cognitive development between the groups. So, harder training wasn't better. Interestingly, the groups didn't decline cognitively as much as the researchers had expected. This result must be interpreted with care, but it strengthens the interest in regular physical activity even when the intensity isn't high.

So, how should one exercise?

The research doesn't provide an exact exercise prescription for Alzheimer's. A reasonable benchmark is about 150 minutes of physical activity per week, spread over several days.
